Title IV Federal Student Aid Authorization Form

All students receiving Title IV funds (Federal Pell, FSEOG, Federal Direct Student / PLUS Loans) are required to complete a Title IV Authorization Form. This form provides Wingate University with instructions on how to apply excess Title IV funds to your student account. Federal Regulations require Wingate University to obtain authorization from the student to use Title IV funds to pay anything other than qualifying charges (any charges other than tuition, room and board).

I do not authorize Wingate University to use my Title IV funds to cover non-qualifying charges on my student account. I understand that I will be responsible for paying these charges myself by the semester due date.

Please note: by declining this authorization, Wingate University will be unable to apply any excess Title IV funds to a book voucher on your behalf.
